Massive cavitation of the lung in Hodgkin's disease.

ostium secundum type; sinus venosus type occurring high in the septum miear the orifice of the superior vena cava, commonly associated with anomabus pulmonary venous drainage; and ostium primum type which is frequently associated with a partial or complete cleft in the antero-medial leaflet of the mitral valve and possibly also in the medial cleft of the tricuspi(l valve. In our department, 75 patients with atrial septal defect have been operated upon during the last five years. Fifty-one patients have had isolated defects, 12 were of the sinus venosus type with anomalous pulmonary venous and 12 patients presented the ostium primum type lesions. In eight this was accompamiied by cleft leaflet of the mitral valve. In this series, the reported case is the only one which was of the secundum type associated with cleft mitral valve. Only two similar cases have previously l)ecn reported, one by Pifarr#{233}1 and the other by Bilhings.m#{176}This emphasizes again the extreme rarity of this condition. As a matter of fact the main sign that led to the suspicion of mitral lesion in those cases was the holosystohic murmur heard at the apex. This could he proved by selective left ventriculography, but since closure of atrial septal defect requires an open heart technic where the mitral and tricuspid valves can be well visualized, it is not always mandatory to perform this procedure.
